If You're A Follow-up Patient

If you're a follow-up patient, you will be sent a reminder appointment card about a month prior to your scheduled visit. On the day of your appointment, simply check in at the front desk, where we will verify your insurance, obtain your referral, and ask you if there have been any changes to the information on your demographic sheet. After making any necessary changes, we will invite you to make yourself comfortable while you wait to be seen.
