First Appointment

After you schedule an appointment for an initial consultation, you will receive a letter verifying the appointment date and time and the name of your provider. Along with the letter, you'll receive:

• a form (called a "demographic sheet") that asks for your name, address, phone number, insurance, referring physician, and address of the physician to whom we will send copies of notes
• an assignment sheet (to have your signature on file for insurance filing; these are obtained annually)
• directions to Mercy Medical Center

Depending on the nature of your concerns, you may see the nurse practitioner and/or the physician.

When you arrive at The Hoffberger Breast Center, you will check in at the front desk, where we will copy your insurance card and insurance referral form. You may then relax in the comfortable waiting area until we call you to be seen.
